For those not fortunate to be in our local servicing area, Goodman TV accepts equipment shipped to our repair depot via all national carriers. We do have an In Shop Estimate Fee of $30.00 plus Tax. This estimate fee does apply towards the repair.
(Customer is responsible for return shipping charges.)

Include with shipment: A copy of this form, a copy of the Proof Of Purchase or a copy of your Contract if under warranty, a money order or cashiers check for $32.48 if out warranty, Battery and AC adapter when applicable.
